| 2. Injury Reports from a Supervisor and the Injured Worker (English or Spanish) are due to CBR within 24 hours of incident.
IMPORTANT: In the rare case your location is outside the service areas of these facilities, you have different protocols and have been notified of those. Click here if you do not know where your protocols are established. |

If there are no NextCare or Concentra locations in your area, different protocols have been established for your company and you have been notified. Where possible, we’ve included maps/locations below.
